The Community Home Based Care (CHBC) initiative is a beacon of compassion and transformation, operating under the auspices of the Global Fund HIV, TB, and Malaria program since 2012. This distinct project, supported by the National Association of People Living with HIV/AIDS (NAP+N), is a testament to our unwavering commitment to bringing essential healthcare services directly to the doorsteps of those in need.
Our dedicated team, known as Team Suruwat, engages in direct service delivery, reaching out to individuals in their communities with a comprehensive range of support. From providing essential medication, on-site coaching, and counselling to nutritional support for children living with HIV, we ensure holistic care. Moreover, women living with HIV receive dignity kits, enhancing their well-being and dignity.
The impact of the CHBC project is profound, bridging the gap between healthcare facilities and individuals in their communities. We have successfully served over 2000 individuals, delivering medical care, emotional support, education, and empowerment tailored to each person’s unique needs.
